Jacky Dale Cook, 58, Phil Campbell, passed away Wednesday, Aug. 1, 2012, at his residence.

Funeral services will be at 11 a.m. Saturday, Aug. 4, 2012, at Spry Memorial Chapel.  Officiating at the service will be Bro. Orbie Pierce. Burial will follow in Liberty Hill Cemetery, Phil Campbell.

He is survived by his children, Kay Lynn Marbutt and Michael Crane; grandchildren, Nick Marbutt and Savannah Marbutt; brothers and sisters, Inez Yealock, Bobby Cook and wife, Hazel, Billy Cook and wife, Connie, Lloyd Cook and wife, Sue, Fayrene Morgan and husband, Dent; and several nieces and nephews.

He was preceded in death by his parents, Oscar and Nancy Cook; sisters, Louise Crane, Polly Cook, Jean Matthews, Winfoline Borden; and brother, Grady Cook.

The pallbearers will be Nick Marbutt, Savannah Marbutt, Cody McDougle, Ricky McDougle, Dalton Newell, Daniel Bradford, Brandon Mitchell and Michael Crane.

The family would like to extend a special thanks to Helen Keller Hospital and all the 4th floor nurses for their love and care shown to us.
